Bob's comments about Apache are spot on. But I would start my search at
http://hotscripts.com. Also, I would install PHP and MySQL if they
weren't already on your server.

Alternatively, you could use the defacto standard: majordomo. But
that's PERL-based, I find PERL less readable than PHP. But that's just
me...
