I'm running squid-2.5.STABLE1-transparent and squidguard-1.2.0 with
db-3.3.11 and it's been good for a few months now. OpenBSD 3.3-stable.
Here are some settings from my squid.conf:

httpd_accel_host virtual
httpd_accel_port 80
httpd_accel_with_proxy on
httpd_accel_uses_host_header on
