Reading the question again.

No, the binddn and bindpassword is correct. You get a very annoying error
message when tese are wrong.

Maybe, but it depends on what arguments you used for squid_ldap_auth.

If squid_ldap_auth is used in search mode then the exact same filter needs
to be specified as user filter to squid_ldap_group. See the -F option in
the manual.
