You could try the "reload_into_ims on" option, I have found this gives my
cache hits a huge rise (50-60% instead of 5-10%) and it doesnt serve up stale web pages which is ideal for me. Combine this with something like adzapper and it will really reduce external traffic for you. While I am aware that the "reload_into_ims on" option technically breaks HTTP, I thinks its probably a little like not returning ping, and doesnt do anything bad, although I could be wrong as I havnt explored every option, however I would like to hear if its caused anyone else problems.
