How to get internal callback

I'm writing dynamic module.
May I get callback from snmpd on each change of some counter? (For
example of disk status or cpu average).

How can I internally(from dynamic module) to query about some counter?
(Then I will query each minute about status of this counter)
Some small example will be appreciated.
