When I was at a similar point in my postgrey installation, I came to
the, ah, rather embarrassing realisation that I had
permit_auth_destination in smtpd_recipient_restrictions before *any* of
my RBL or policy checks, which rendered them all pretty much impotent.

YOu might want to check for something similar...
