Postfix has two programs that deliver to mailboxes: local(8) and virtual(8). In recent Postfix versions, both understand address extensions. Neither of the two will name mailbox files after the extension. That would be a terrible security problem where miscreants could pollute the mailspool with lots of junk file names.
