This has nothing to do with Postfix or its configuration. You should go
through your Courier authdaemon configuration: authdaemonrc and
authmysqlrc. Check also your logs for related error messages. Read
through the Courier documentation once more.

If you have enabled digest or CRAM authentication you should have
_cleartext_ passwords in your MySQL tables.

You should start with a simple configuration and make it work. Then you
can add complexity (MySQL tables, challenge authentication, SASL) one
feature at a time. This way you can always go back to some working
configuration and you also know what is not working.
