All I was looking for is some pointers to get me started debugging this
thing. It used to work, but the all-users list gets used so rarely, that
I don't know what has changed in the meantime.

As I read the docs, vscan_default_recipient_limit causes the queue
manager to send multiple copies of the message to the vscan transport,
each copy has, at most, vscan_default_recipient_limit recipients.
However, there seems to be another limit somewhere, since if I increase
vscan_default_recipient_limit, then all the recipients get bounced.
