Must be DNS. The ident timeout is 30 seconds, sharp. This could be anything:
forward or reverse DNS lookup on the IP address, or blacklist lookups, if
you are using blacklists.

That's also possible if you've maxed out on the maximum number of SMTP
connections. If so, though, you should see warning messages in syslog.
