I use courier-mta with MySQL for 10,000+ accounts. It works very well. I
also use MySQL for SpamAssassin and SquirrelMail settings so having
everything in one place is very convenient for scripting and the like.
I've doc'd my setup at

The docs are pre-authlib but should still give you a reasonable idea of
how things work.
