In my experience, this should work out of the box, at least on sarge. Try "courierauthtest USERNAME" to check if the authentication works. Check that the maildirs exist; if they don't, instruct the users to log in, go to their homes and execute "maildirmake Maildir", or do it as root but remember to chown the maildir as appropriate.
