experimental patch for courier (version 4.0.1).

It tries to do what tomfa's courier 1.6.1 patch did but a different
approach.

I don't use very feature of courier so don't know how it effects some
features, but I run it on my own personal courier server and it works
fine. The patch was put up now in response to this email, I may have
inadvertly left some stuff out as I had to remove a whole load of extra
stuff I added to the code that no-one will finding interesting except me.

Yes I know the arguments about obeying namespace etc (I have also read
thunderbirds argument why toplevel doesn't work on courier) .... but
those who use my server just want it to look like how cyrus-imap
displays it. They don't want to change IMAP client or play with settings.
