The last time I checked (last week) almost all the missing "autolearn=..." were due to a cached result, as the same spam contents was hitting lots of our recipients. Increase the $log_levels and search for: "spam_presence cached, skipping spam_scan" (at loglevel 5). Also the absence of "SA check:" in a TIMING report at log level 2 indicates the SA was not called.
