Are you overriding default notification templates?
Perhaps you have templates from 2.4.0 - see 2.4.1 release notes
about incompatibility due to ccat renumbering.

On our site a sender of banned contents gets:

Subject: BANNED message from you (multipart/mixed | ...
....
BANNED CONTENTS ALERT

Our content checker found
banned name: multipart/mixed |
application/x-msdownload,.empty,Attachments00.HQX
