Depends on the patch and the password-checker backend.
Over here CRAM-MD5 for SMTP-AUTH works with vchkpw w/ clear password
enabled.

Modify the patch, repatch, recompile and reinstall qmail.

The Bat! uses the most secure authentication method announced in EHLO
response. As CRAM-MD5 is more secure than LOGIN and PLAIN The Bat! will
always try to use it.
