The mdaemon site is only waiting 30 seconds for qmail-smtpd to start
talking after the connection is initiated. That's not very
long. However, your qmail-smtpd should start talking almost
immediately. See:

the SMTP protocol is very slow to start talking, so this could be the
problem!

I'll send this to my qmail hoster and try and do something on the mdaemon
server, thanks
