Can EXIM be setup to filter outgoing mail through clamd and/or through
spamassassin (with a threshold for refusing to send it) ?

It can. See the exim docs for more information on how to make exim and
clamd work together.

You should take care which ACLs you configure as you don't specify
what you mean with "outgoing mail". Does exim receive these mails via
SMTP?
