I think your problem is that the smtp deamon is not running...

/service/qmail-smtpd: up (pid 12677) 0 seconds

the seconds should be greater than one. try to shut down the service, then
restart it.
svc -d /service/qmail-smtpd

then
execute /service/qmail-smtpd manually. see if you get an error..

then ctrl C and
svc -u /service/qmail-smtpd

to bring the service back up
